Step One — Enroll

Should you choose to pre-fill it prior to printing, please ensure you sign it and circle the answers in the highlighted sections. Then email the completed form along with your employee listing to us at staff@collectionplus.com.

I AM A FMCSA/FTA regulated employer with 6 or more drivers
I AM A FMCSA regulated employer with 5 or less drivers
I AM A FMCSA regulated OWNER OPERATOR
I AM NOT a federally regulated employer (i.e. construction)
I am regulated by the California Public Utilities Commission (PUC) and have an assigned ID Number.
I am a towing company working with the California Highway Patrol (CHP) and need to be in a CSAT program.
OUR POLICY
Just a few reminders:
- We select for random testing each quarter. You have the chance of being selected up to four times a year.
- You are required to come in for testing within three weeks of the company being notified.
- Failure to come in for testing within 3 weeks or failure to communicate your status (out of the country) may result
in dis-enrollment from the consortium with no refund.
We will provide you with all the necessary reports and results throughout the year. PLEASE MAKE SURE TO KEEP US UPDATED ON CHANGES to any contact information. Also, as a reminder, you should always have the following items available for a BIT Inspection. If we must reproduce any items there is a $50.00 charge and a two working day lead time.
- Annual Enrollment proof letter
- Proof of any FMCSA data queries
- Any employer copies of Drug and Alcohol Testing Forms
- Any drug test results
- Mid-year and End of Year Reporting
Step Two — Payment
We will invoice you via QuickBooks and you can pay online, or mail in a check.
Step Three — Receive Enrollment
We will email you, and mail you a copy of your Proof of Enrollment Letter.
